Medical Marijuana Certification
Provides a medical marijuana card certification to individuals who have a medical condition that the provider deems can benefit from medical marijuana.
What is a medical marijuana card?
A personal identification card that shows the patient has a medical condition and is registered with Virginia Board of Pharmacy. With the card patients can buy medical marijuana at Virginia dispensaries.
Qualifications:
•Virginia resident
•Twenty-one years of age
•Qualifying medical condition
If medically approved: registration through Virginia board of Pharmacy dhp.virginia.gov is required.
To Schedule
the following is needed
A primary care physician referral
New patient paperwork completed
$100 deposit to hold the appointment (the deposit covers the appointment)
