New Patients

  • Patients with scheduled appointments by another provider’s office are required to complete all the electronic paperwork one-week prior to scheduled appointment.

  • Self-referrals and parental referrals are required to complete electronic paperwork prior to scheduling appointment. 

*This allows the provider ample time to review information and check insurance coverage. *

  • This paperwork is located in the patient portal account. 

  • If you do not have an email or internet access, please let us know prior to your appointment so arrangements can be made. 

  • If appointment is not confirmed and paperwork is not completed 48-hours prior to the appointment, it will be canceled by the office. 

  • Appointments can be confirmed or canceled through the text, email or phone call reminders. If the appointment is canceled through the reminder you will need to call the office to reschedule.
