New Patients

If you call our office and receive no answer, please leave a voicemail. We do not have a receptionist, however during business hours a provider is here and will return your call within 72 hours.

1. We need a referral from your primary care physician (typically this is faxed to 276.880.0036).

2. When we receive the referral our staff will contact you to give access to your patient portal account.

3. In the portal you will complete the new patient paperwork (if you do not have an email or internet access  other arrangements can be made).

4. When the paperwork and authorization with your insurance is completed staff will call you to schedule your initial appointment.
